| Step 2 (Visit Local Acceptance Agent) 1 2 3 |
Obtain application authorization from an Acceptance Agent.
Bring materials listed in Step 1, including US Government payment to a Passport Acceptance Facility near you and notify the agent that you will be using an expediting service to "hand carry" your application.
Children 15 years and younger, Both parents must appear with the child at the acceptance agent when obtaining the sealed envelope. If both parents cannot be present then a notarized Written Statement of Consent form (DS-3053) must be provided to the acceptant agent.
The acceptance agent will take about five to ten minutes to go through the following steps:
-
To verify your identity, the agent will request your proof of identity.
-
The agent will ask you to sign the DS-11, and then the agent will sign and seal it.
-
To verify that you need an expedited US passport, you will need to show them your travel documentation.
-
The agent will then seal the DS-11, photos, proof of citizenship, travel documents, authorization letter and associated government fees in an envelope marked "To be Opened by US Passport Office Only".
Note: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".
IMPORTANT:
- Do not open this sealed envelope or the application will be invalid.
- Please write the Applicant Name / Date of Birth / Departure Date on the outside of the sealed envelope and
- Attach one copy of the unexecuted DS-11 application, completed as required in Step 1A.
Please Note: The sealed envelope is valid for only five business days. After completing your order request, please forward sealed envelope to National Passport.com for immediate processing.
