Step 1 (Assemble passport materials) 1 2
A) Completed Second Valid US passport application
B) Current Valid US Passport
Your valid US passport must have at least one year of validity left. If your passport is about to has less than one year of validity, you will need to renew it before you appy for a second valid passport.
C) Request Letter
Please submit letter addressed to the "State Department" requesting a second valid passport. The letter must explain in detail the reason for your request.
C) Two Passport Photographs
- 2x2 inches in size

Identical

Taken within the past 6 months, showing current appearance

Color
-
Full face, front view with a plain white or off-white background

Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head

Taken in normal street attire

Uniforms should not be worn in photographs except religious attire that is worn daily.

Do not wear a hat or headgear that obscures the hair or hairline.

If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture.

Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons.

A medical certificate may be required.

You must provide a copy of ONE of the following :

your foreign travel plane ticket

your printed travel itinerary showing foreign travel

If this is a business trip, you may present a business letter from your employer. Click here to view a sample letter.
F) US Government Fee:
- Make a check or Money Order for $135 payable to U.S. Department of State.

Be sure to include your date of birth on the check

Starter Checks and 3rd Party Checks will NOT be accepted.
