A) Completed US passport application
-
- If you are applying because your most recent, valid passport was lost or stolen, you will also need to submit Form DS-64.
B) Proof of US Citizenship
If you were born in the United States you may provide one of the following:
- Certified US Birth Certificate issued by city, county or state
- Previous US Passport
If you were born outside of the United States you must submit an official copy of one of the following:
- Previous US Passport
- Certificate of Naturalization Certificate
- Certificate of Citizenship
- Consular Report of Birth Abroad (Form FS-240)
- Certificate of Birth (Form DS-1350 or FS-545)

C) Proof of Identity
- Copy of current, valid Driver's License with Photo (must be issued greater than 6 months ago
- If a Driver's License isn't available, then you need to supply an alternate proof of identity. Please click here for further information.
D) Two Passport Photographs
- 2x2 inches in size
- Identical
- Taken within the past 6 months, showing current appearance
- Color
- Full face, front view with a plain white or off-white background
- Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
- Taken in normal street attire
- Uniforms should not be worn in photographs except religious attire that is worn daily
- Do not wear a hat or headgear that obscures the hair or hairline
- If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture
- Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons. A medical certificate may be required.

You must provide a copy of ONE of the following:
- your foreign travel plane ticket
- your printed travel itinerary showing foreign travel

G) US Government Fee:
Age 16 and older: Passport fee $110.00
Execution fee $25.00
Government expedite fee $60.00
Total $195.00
Under Age 16: Passport fee $80.00
Execution fee $25.00
Government expedite fee $60.00
Total $165.00
Note: When applying at an authorized Acceptance Facility and paying by check or money order, these fees must be paid separately because the $25.00 Execution Fee is retained locally by one of 6,500 designated passport application acceptance facilities across the U.S. (Acceptance facilities may or may not accept credit cards.)
