Passport must not be damaged

You must provide 2 copies of your valid driver's license or state ID card, issued more than 6 months ago.

If your ID was issued less than six months ago, please click here for alternate proof of Identity.

• 2x2 inches in size
• Identical
• Taken within the past 6 months, showing current appearance
• Color
• Full face, front view with a plain white or off-white background
• Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
• Taken in normal street attire
• Uniforms should not be worn in photographs except religious attire that is worn daily.
• Do not wear a hat or headgear that obscures the hair or hairline.
• If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture.
• Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons.
• A medical certificate may be required.

Print and fill out Authorization Letter.

E) Proof of Departure

You must provide a copy of ONE of the following :  

• your foreign travel plane ticket
• your printed travel itinerary showing foreign travel
• If this is a business trip, you may present a business letter from your employer. Click here to view a sample letter.

• Make a check or Money Order for $60 payable to U.S. Department of State.
• Be sure to include your date of birth on the check
• Starter Checks and 3rd Party Checks will NOT be accepted.
