Step 1 (Assemble US Passport Materials) 1 2 3

A) Completed US passport application

Complete the DS-11 application form online (required for expedited services).
- Please print two copies of the completed DS-11.
- Fax (202.315.3807) or E-mail (info@nationalpassport.com) copy of electronically created DS-11 form.
Note:Do not sign the DS-11 application until instructed to do so by the Passport Acceptance Agent.

B) Proof of US Citizenship

If you were born in the United States you may provide one of the following:

Certified US Birth Certificate issued by city, county or state
Previous US Passport

If you were born outside of the United States you must submit an official copy of one of the following:

Previous US Passport
Certificate of Naturalization Certificate
Certificate of Citizenship
Consular Report of Birth Abroad (Form FS-240)
Certificate of Birth (Form DS-1350 or FS-545)

C) Proof of Identity

Copy of current, valid Driver's License with Photo (must be issued greater than 6 months ago

D) Two Passport Photographs

2x2 inches in size
Identical
Taken within the past 6 months, showing current appearance
Color
Full face, front view with a plain white or off-white background
Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
Taken in normal street attire
Uniforms should not be worn in photographs except religious attire that is worn daily
Do not wear a hat or headgear that obscures the hair or hairline
If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture
Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons. A medical certificate may be required.

E) Letter of Authorization

Print and fill out Authorization Letter (Minors).
Note: Children 15 years and younger: Both parents must appear with the child at the acceptance agent when obtaining the sealed envelope. If both parents cannot be present then a Written Statement of Consent form (DS-3053) must be provided to the acceptant agent.

F) Proof of Departure

You must provide a copy of ONE of the following:

your foreign travel plane ticket
your printed travel itinerary showing foreign travel

G) US Government Fee:

Age 16 and older: Passport fee $75.00
Execution fee $25.00
Government expedite fee $60.00
Total $160.00

Under Age 16: Passport fee $60.00
Execution fee $25.00
Government expedite fee $60.00
Total $145.00

Note: When applying at an authorized Acceptance Facility and paying by check or money order, these fees must be paid separately because the $25.00 Execution Fee is retained locally by one of 6,500 designated passport application acceptance facilities across the U.S. (Acceptance facilities may or may not accept credit cards.)
