Check or Money Order for $60 payable to U.S. Dept of State.

• Starter Checks and 3rd Party Checks will NOT be accepted.
  

D. Proof of Departure

• printed travel intinerary or,

E. Letter of Authorization

• Print and fill out Authorization Letter.
