All Children must travel with their own passport: (15 Years Old and Under)

You are applying for a Child US passport for the first time or previously issued Child US passport was only valid for 5 years or less.
Children 15 years and younger: Both parents must appear with the child at the acceptance agent when obtaining the sealed envelope. If both parents cannot be present then a notarized Written Statement of Consent form (DS-3053) must be provided to the acceptant agent.
