New US Passport
·You are applying for a US Passport for the first time
·You do not have your expired US Passport in your possession
·Your US Passport has expired and was issued more than 15 years ago
·Your previous Passport was issued when you were under 16 yrs and you are now over 16
If you meet any of the above criteria, you can follow these instructions to obtain your expedited US Passport.
Renew US Passport
·Already have a US passport that is not damaged;
·and Received it within the past 15 years;
·and Were over age 16 when it was issued;
·and Still have the same name as in passport or
*you can legally document your name change*
NOTES:If you mutilate or alter your US passport, you may invalidate it and risk possible prosecution under the law (Section 1543 of Title 22 of the U.S. Code). If your US passport has been mutilated, altered or damaged, you cannot apply by mail. You must apply in person as an New Applicant.

All Children must travel with their own passport.

To replace a damaged passport for applicants age 16 and over.
Amend US Passport
A US Passport Amendment can be requested for making a data change for a previously issued, currently valid U.S. passport WITHIN one year from the date of issue:
When there is a change in your identifying information, such as a name-change due to marriage or by court order;
To extend the validity of certain previously limited passports; or
To correct a Passport Services’ printing error.
*If it is MORE than one year since the date of issue, you must apply for a New (DS11passport or a renewal

Second Valid US Passport
Select this process if you currently have a valid US passport and need to apply for a second one for security or other reasons.
Requires a letter stating your reason for a second passport
Extend Validity of Limited US Passport
You can extend the validity of your passport if:
You still have the limited passport in your possession and it is not damaged, And
You can provide the documents that were missing the time you were issued the 1year passport, And
Your limited passport is extendable* and was issued less than 1year ago.
*A non-extendable passport will read "this passport is not extendable"on one of visa pages.
